Loughview Health Membership Terms & Conditions

1. Annual Subscription Services

1.1 Individual Membership:
a) Members are entitled to up to 12 face-to-face Standard GP appointments during a 12-month period.
b) Unused appointments cannot be carried over to the following membership year.

1.2 Family Membership:
a) Members are entitled to up to 24 face-to-face Standard GP appointments during a 12-month period.
b) Unused appointments cannot be carried over to the following membership year.
c) Family membership includes up to two adults and two children under the age of 23.

1.3 Service Limitations:
Loughview Health reserves the right to withdraw membership or recommend alternative care pathways if the level of care required exceeds what is reasonably covered under the membership or if the member requires services that Loughview Health does not provide.

1.4 Appointment Duration:
Standard GP appointments are scheduled for 15 minutes. Membership does not include Specialty clinic appointments

1.5 Operating Hours:
Loughview Health is open from Monday to Friday, 9:00 am to 6:00 pm, and Saturday, 9:00 am to 1:00 pm. Early morning and late evening appointments are available on Mondays and Wednesdays (7:30 am-9:00 am and 6:00 pm-7:00 pm). Please note that Loughview Health will be closed for one week during the Christmas holidays. Operating hours may vary during other public holidays and are subject to change without prior notice due to unforeseen circumstances.

1.6 Private Prescriptions:
Private prescriptions may be issued during consultations. While there is no additional fee for the prescription, pharmacies may charge dispensing fees for private prescriptions.

1.7 Medical Supplies and Testing:
Routine medical supplies are provided at no additional cost. Vaccinations and laboratory tests will incur additional charges, which will be communicated to you at the time of your appointment.

1.8 Appointment Availability:
We aim to offer GP appointments within two working days of receiving your request. Please note at times we may not be able to accommodate due to GP availably ( ie annual leave of practitioners, Sickness absence) if this is the case we aim to offer the next available appointment

1.9 Health MOT Discount:
The 10% discount on Health MOTs is available once per year, per adult.

2. Pricing & Payment

2.1 Membership fees can be paid monthly or annually via direct debit or annually by credit or debit card.

2.2 Failure to make payment by the due date will result in suspension of your membership until payment is received.

2.3 Members may cancel their membership and receive a full refund within 14 days of registration, provided no membership benefits have been utilised.

2.4 The initial membership period is 12 months. No refunds are available for cancellations made after the 14-day cooling-off period. If benefits have been used within the 14-day period, the cost of services will be deducted from any refund. If the cost of services exceeds the subscription payment made, the member will be liable for any additional charges.

2.5 Services not covered by the membership must be paid for in full at the end of each appointment. An invoice and receipt will be provided upon request for insurance or reimbursement purposes.

2.6 Fraudulent use of the membership will result in immediate termination without refund, and the member may be held liable for the cost of services accessed fraudulently.

2.7 Memberships are non-transferable and cannot be assigned to another individual.

2.8 Membership benefits and services may change from time to time. We will provide notice if changes are made, and where necessary, offer an equivalent service.

3. Term & Termination

3.1 These terms apply for the duration of the initial 12-month membership period, and for any continuation period unless terminated by either party.

3.2 You may cancel your membership within 14 days of its commencement date.

3.3 We will notify you two months before the end of the initial membership period to inform you about renewal options.

3.4 Loughview Health reserves the right to increase membership fees following the initial membership period. Members will be notified of any price changes two months in advance.

3.5 After the initial 12-month membership period, you may cancel your membership with 30 days' written notice. If services paid for in advance are unused, we will provide a pro-rata refund.

3.6 Loughview Health may terminate the agreement with 30 days' written notice if services can no longer be provided. In this case, a pro-rata refund will be issued for any unused services.

3.7 Members may terminate the agreement immediately if:
a) Loughview Health breaches the terms and fails to correct the issue within 20 working days.
b) Loughview Health enters liquidation or administration.
c) Changes are made to the terms of the membership that disadvantage the member.
d) Services are unable to be provided due to events beyond our control.

3.8 Loughview Health may terminate membership immediately if:
a) Payment is not received on time.
b) The member breaches the terms and does not rectify the issue within 20 working days.
c) The member fails to attend three consecutive appointments without prior notice.

4. Appointments & Cancellations

4.1 Members can cancel appointments without penalty. However, cancellations made within 24 hours of the appointment or missed appointments may be deducted from the annual allocation of GP appointments.

5. Liability

5.1 Loughview Health is only liable for foreseeable losses or damages that directly result from a breach of these terms. We are not responsible for unforeseeable losses or damages.

5.2 We do not exclude liability for:
a) Death or personal injury resulting from our negligence.
b) Fraud or fraudulent misrepresentation.
c) Breaches of implied terms relating to satisfactory quality and fitness for purpose.

7. Data Usage

7.1 We may use your contact information to provide details about services or offers of interest.

8. Miscellaneous

8.1T his membership is not an emergency or an out of hours service and does not replace serviced provided by NHS paramedic services of NHS GP

8.2  We recommend members to remain registered with an NHS doctor throughout your membership period.

8.3  Whilst we are responsible for administrating the scheme, we cannot be held liable in any way whatsoever for injury, loss or damage arising directly or indirectly from the advice given by any third party associated with the scheme.

8.4  We may transfer our rights and obligations under these terms to another organisation and we will always notify you in writing if this happens but will not affect your rights or our obligations under these terms.

8.5  This contract is between you and us, no other person shall have any rights to enforce any of its terms.

8.6  Loughview Health LTD clinicians hold adequate professional indemnity insurance to cover for the acts and omissions and is committed to practice withing the statutory requirements in the UK and to best clinical practice guidelines.

8.7  f we fail to insist that you perform any of your obligations under these terms, or if we do not enforce our rights against you , or if we delay in doing so that will not mean that you do not have to comply with those obligations. If we do waive a default by you, we will only do so inwriting and that will not mean that we will automatically waive any later default by you

8.8  The Terms of this contract are governed by, and shall be construed in accordance with, the laws of Northern Ireland. Any litigation arising from this contract will be conducted under the exclusive jurisdiction of the courts of Northern Ireland.
